Please download and complete the following new client paperwork and bring it with you to your first appointment.

Childhood History Form (for children or teens living with their parents)

Adult History Form (for young adults living independently)

In addition to the above paperwork, please bring with you any previous evaluation or treatment records or reports, relevant health records, and school records (IEP, 504 Plan, school testing records). Please contact your child’s school to obtain your child’s most recent school records if you do not already have a copy.
